What Services Do We Offer?

Comprehensive and Targeted Medication Reviews will allow us to examine your current medications and make sure they are safe for you. We will answer any questions or concerns that you may have about your medications, including any possible lower-cost alternatives. If needed, we will work with you on ways to keep track of your medications and send you a summary of what was discussed.

  • Annual Comprehensive Medication Review (CMR):  A yearly check-up of all your medications with a pharmacist. You will get a written summary of this review.
  • Quarterly Targeted Medication Reviews (TMRs):  Regular checks to ensure your medications are working well for you.
  • One-on-One Help:  You can talk to a pharmacist in person or over the phone about your medications.
  • Safe Disposal Information:  Each year, we will give you information on how to safely dispose of certain prescription medications.

How Does Enrollment Work?

You will be automatically enrolled in the MTM program if you qualify, but you can choose to opt-out if you prefer. We will check each quarter to see if you still qualify.

What documents are needed?

  • Be prepared by having a list of your current medications. You can find a sample medication list here (attach document).
