Our Medication Therapy Management Program (MTMP) is focused on improving therapeutic outcomes for Medicare Part D members.
To qualify for MTM, a member must meet all of the following criteria:
Members must have filled eight or more chronic Part D medications; and
Members must have at least three of the following ten chronic conditions:
- Alzheimers' Disease
- Bone disease-arthritis (including Osteoporosis, Osteoarthritis, and Rheumatoid Arthritis)
- Chronic Congestive Heart Failure (CHF)
- Diabetes
- Dyslipidemia (High Cholesterol)
- End Stage Renal Disease (ESRD)
- Human Immunodeficiency Virus/Acquired Immunodeficiency Syndrome (HIV/AIDS)
- Hypertension (High Blood Pressure)
- Mental Health (including Depression, Schizophrenia, Bipolar Disorder, and other chronic/disabling mental health conditions)
- Respiratory Disease (including Asthma, Chronic Obstructive Pulmonary Disease (COPD), and other chronic lung disorders
Members must be likely to incur annual costs of $1,623 for all covered chronic Part D medications.
Are in a Drug Management Program to help better manage and safely use medications such as opioids and benzodiazepines
What Services Do We Offer?
- Annual Comprehensive Medication Review (CMR): A yearly check-up of all your medications with a pharmacist. You will get a written summary of this review.
- Quarterly Targeted Medication Reviews (TMRs): Regular checks to ensure your medications are working well for you.
- One-on-One Help: You can talk to a pharmacist in person or over the phone about your medications.
- Safe Disposal Information: Each year, we will give you information on how to safely dispose of certain prescription medications.
How Does Enrollment Work?
You will be automatically enrolled in the MTM program if you qualify, but you can choose to opt-out if you prefer. We will check each quarter to see if you still qualify.
