To enroll in the Healthy Mississippi (HMO) Plan, you will need to provide your current Medicare Member Number (found on your Medicare card), phone number, and your permanent address. Please ensure that you fill out all the sections labeled as “Required”. The remaining sections are optional, and your coverage cannot be denied if you choose not to complete them.

Enroll Online Through Medicare.gov

Finding the Right Plan for You

Healthy Mississippi’s plan serves to bridge the disparities and address expenses not included in your Original Medicare Part A & Part B programs. You qualify for our plan if:

  • You live in our Geographic Service Area
  • You are a United States citizen (or lawfully present in the US)
  • You have both Medicare Part A & Part B

Do You Have Chronic Conditions?

You are eligible to join the Healthy Mississippi plan if you have been diagnosed with any of the following conditions:

  • Diabetes Mellitus (High Blood Sugar)
  • Chronic Heart Failure (CHF)
  • Cardiovascular Disorders - must be one of the following:
    • Cardiac Arrhythmias
    • Peripheral Vascular Disease
    • Coronary Artery Disease
    • Chronic Venous Thromboembolic Disorder

Part A of Original Medicare covers in-hospital stays and services, skilled nursing care, hospice care and certain types of home healthcare.

Part B of Original Medicare covers doctor visits, outpatient care, other types of home healthcare, medical equipment and many preventive health services.
